Leave Sharing Policy - 5406
Leave Sharing Request Form - Shared leave may be requested if the requesting employee has been called to service in the uniformed services, or suffers from, or has a relative or household member suffering from, an illness, injury, impairment or physical or mental condition which is of an extraordinary or severe nature as defined by WAC 357-31-390; and which has caused, or is likely to cause, the employee to:
• go on leave without pay; or
• terminate employment; or
• deplete his/her paid leave reserves; or
• be ineligible for benefits under RCW 51.32 (Industrial Insurance).


Acceptable Use Policy (AUP) - Electronic Information Networks Individual User Access Informed Consent Form for all staff, volunteer and community users of District Technology accounts and/or systems. 


Key Agreement Form - Emplyee/Coach Key Agreement Form, required for all users issued ASD5 access keys.

Volunteer Forms

Thank you for offering your services to volunteer in the Aberdeen School District. It is required to annually complete a volunteer application.

Please complete the following steps:

  1. Download the form(s).
  2. Fill out the form completely, including all activities being requested to volunteer for and the anticipated dates of the activity in which you would like to participate, and providing all personal information requested on application.
  3. Please disclose all past convictions. Failure to disclose this information may result in a denied application.
  4. Once completed, please return the application to Human Resources in the District Office, Attn: Julie Minsker or you may drop off your application in a sealed envelope at any of our schools.
  5. If your application is not complete, Human Resources will notify you.
  6. Once your application has been processed, the building principal and secretary will be notified of the results.
